Primary Care Network (PCN) is a team-based healthcare model in BC that strengthens primary care by connecting doctors, nurses, and allied health professionals to better serve patients.
PCN is a provincial initiative from the Ministry of Health, Divisions of Family Practice, Doctors of BC Medical Association as well as the health authorities (in the case of the Coast, VCH), and is in local partnership with the shíshálh Nation and Pender Harbour Health Centre.
OBJECTIVE: To design a suite of office use assets and marketing materials for PCN, a coordinated healthcare model that aims to improve access to care for residents of the Sunshine Coast.
